Does your student want to try out for a sport in 5th, 7th, or 8th grade?
If so, YOUR child MUST have a sports physical on file before tryouts!
MEDICATIONS: A physician's order must ACCOMPANY ALL MEDICATIONS to be given during the school day, including over-the-counter drugs. If a student must receive medications in school (including students in need of emergency medications such as Epi-Pen, inhalers, Benadryl, Diastat) a physician order is REQUIRED and must be obtained prior to administering medication. Please note that a NEW ORDER is required at the beginning of EVERY school year. A prescription label IS NOT acceptable in lieu of a doctor's order.
NO CHILD is to take or carry PRESCRIPTION OR OVER THE COUNTER MEDICATIONS at school. All medication must be brought into school and must be appropriately labeled. Please be sure to ask the pharmacy for a second prescription bottle with a label for school.
OVER-THE-COUNTER MEDICATIONS: These require a medication form to be filled out every single year if you would like your child to receive these medications at school. These type of medications would be Tylenol, Ibuprofen, and Benadryl. You will provide a bottle to be kept at school and it will be labeled with your student's name and grade.
