How to Get Your Prize

Below, you can find a copy of our Google Form, which is where you can submit each milestone you have achieved, and pick your prize! This form needs to be filled out for every milestone you have reached. Follow the prompts in the form to claim your prize - descriptions and pictures can be found on our "Prizes" page! (Pictures are also found throughout the Google Form!) 

You can pick up your prizes on the day of your choosing. Please make sure to give us at least one (1) full business day (weekends are not included) to get your prize ready for pickup. If you submit the form on:
*Monday, it will be ready Wednesday

*Tuesday it will be ready Thursday

*Wednesday, it will be ready Friday

*Thursday, it will be ready Monday

*Friday, it will be ready Tuesday

*Saturday or Sunday, - will be ready Tuesday

Do not feel you need to rush in to pick up your prize. If you cannot come in as soon as you submit the form, that is completely fine! Just note in the "other" section below which date you want to come. Keep in mind the time slots will remain the same for each day of the week.

Once you pick the date and time you wish to pick up your prize, head to the library at 150 Park Ave during the allotted time slot. During the hours the library is open for walk-in service, you can come on in and get your prize by coming to the main desk. Or, if you choose to, you can fill out the form below and you can just run in and grab your prize. We have a dedicated shelf in the library (see image below), near the main desk, where you can find your prizes.  There will be a bag with your name on it and the prize you chose. Please make sure you take only your bag =)
