The PTP is required for ALL students aged 13 who will turn 14 during the life of the active IEP and annually thereafter.
School Readiness - For students who are considering college, a technical school, or formal training program such as an apprenticeship.
Career Interest - For students who are undecided about what direction they'd like to take or who have not declared an academy.
Employability Skills - For students who will struggle to exhibit appropriate work skills (e.g. social, communication, timeliness, work speed, work quality, work productivity, etc.), or those who need help understanding the "rules" of work (e.g. when it's ok to take a break, chain of command, etc.).
Independent Living Skills - For students in all disability areas, used to determine whether independent living goals are needed, and if they are, in which areas.
Student Self-Assessments - For students to reflect on their current skills and situation as related to post-secondary needs. Students complete the assessments with the help of their case manager (if needed).
Parent Questionnaires - For any parent who you'd like to engage more in the IEP process. Helps the parent focus their thoughts/concerns about their child's immediate future, identify areas of need they hadn't considered yet, bring their voice to the table.
Visual Assessments - For students who are non-verbal or low/non-readers and require visual support to answer questions.
