7th Grade Registration Google Form

Parent/Guardian:  Scan the QR Code below and complete this form to enroll your incoming LSMS 7th grader in elective classes. Please note you will need to use your student's RSD email address to access the Google Form.

The form should be completed by only one parent/guardian of each student prior to registering on Infinite Campus.

Once submitted, changes can be made to your response up until the enrollment window has ended on February 19, 2025 at 4 pm.   The RSD student email account used will automatically receive a copy of the responses. If any changes need to be made on the form, simply "Edit the Response" from the link that is emailed. The latest form submitted will be considered the final request.
Requests for changes after the enrollment window has ended will be considered on a case by case basis
and will need to go through the grade level counselor.
