RRPS must follow immunization requirements set by the New Mexico Department of Health.
For new or returning students, please send immunization records via fax or upload to the Confidential Medical Document Upload Form for evaluation by the Health Office to complete registration. All students are expected to have current immunizations or an approved exemption throughout the school year.

Immunization Requirements 2022-2023
All incoming Kindergarteners need the following vaccines:
5 doses of DTAP/DT/TD [Diphtheria/Tetanus/Pertussis * Last dose must be on on after 4th birthday with at least 6 months between the last 2 doses
4 doses of Polio [IPV] * Last dose must be on on after 4th birthday with at least 6 months between the last 2 doses
2 doses of MMR [Measles/ Mumps/ Rubella]
3 doses of Hepatitis B [Hep B]
2 doses of Varicella [VAR]
2 doses of Hepatitis A [Hep A].
Is your child needing to catch up on immunizations? That's OK! You can view the CDC Catch Up Schedule for immunizations. If you are enrolling your student, and they are not up-to-date on shots, you may provide an appointment confirmation letter to the school nurse for temporary approval. Once your child receives their up-to-date shots, please send the School Nurse the updated shot record. Thank you!
If you choose to exempt your child from vaccinations, the State requires an exemption form be completed every school year for each child.
Please read the State Exemption Form carefully. After completion, it must be notarized and sent to Santa Fe for consideration. The form is linked below.
