Immunizations

*If you are not able to immunize your child due to MEDICAL or RELIGIOUS reasons, you must complete the Immunization Exemption Form annually. Any form not completed correctly or in full will be rejected by the state.

*After completion,  it must be notarized and sent to Santa Fe for consideration.

*If you file an immunization exemption, you MUST bring the school nurse a copy of the notarized form.
