506 Form 

If you have not done so already, please complete the 506 form for your child. You will need to click on the link above to print and fill out. Please send back to school with your child and it will get forwarded to the District Office.

Please note that you only need to fill out this form once for the entire time your child is part of Rio Rancho Public Schools.
