The Rootstown Local Schools requires the following information to register your student for attendance at Rootstown Local Schools.

• Proof of Residency - MUST HAVE TWO

  1. Your rental or purchase agreement or mortgage statement AND

  2. Your most recent utility bill (electric, gas or phone)

WE CANNOT TAKE DRIVERS LICENSE, CHECKS OR CREDIT CARDS.

• Child's Birth Certificate

• Child's Immunization Record

• Custody Papers (If Applicable)

• Preschool Only - Income Statement

• Special Education - Need Current IEP
