Required Participation and Physical Forms: In order to try out for a team or participate in any sport, you must have a current RRISD Sports Physical on file and all online forms must be completed. Please be sure to submit all of the online forms.
Athletics Physical Form: All athletes must have an annual sports physical on file in order to participate in athletics or compete/tryout in any sport for the 2024-2025 school year. Please have it completed prior to the start of the new school year. The physical will expire exactly one year after the exam. Be sure to complete all pages including the medical history portion. Please make a copy for your records.
