Forms and Documents

All NJHS members must earn 15 hours of service by the end of April. Students are expected to complete HALF their service by December 2025 and the rest BEFORE the end of April 2026.

Students may begin serving beginning June 1, 2025. Please keep a record of your service and submit the forms at our first meeting in September 2025.

Below is a copy of our Service Form so you can use for your records.  All service forms must be turned in by end of April 2026 to be counted. Remember to fill out and submit your service form within the month you serve.

You may fill out either the Service Hour printable copy (must be turned in to the NJHS adviser). It is your responsibility to keep a record of the service hours earned.

• Absentee Form - only ONE excused absence is allowed per semester.

• For current NJHS members, go to https://www.njhs.us/studentaccount/  to register for a student account. If you are under 13 years of age, your parents must fill out a consent form: https://www.njhs.us/create-an-account/coppa-parental-consent-form/ . Once completed, I will receive an email to verify your NJHS member status. 

• NJHS Photo Parent's Permission Form - to give consent to post NJHS photos of your student in yearbook, Walsh e-news, NJHS website, and social medias.
