School Instrument Usage Form / Payment

Fill out the Instrument Usage Form with the following guidelines:

• Violin / Viola Fee will be $52 for the year.
• Cello / Bass will be $26 for the year - this is for instruments played at school only and shared among other classes.
• Cello / Bass will later pay $26 if they are selected to use a school-owned instrument for home practice.
• Once you have filled out the Instrument Usage Form, you will need to make the payment using the link below the form.
