Required Medical Forms 

• Step 1 - Electronic Participation Forms - Complete online

• Step 2 - Parent Contact/Medical History/Physical Forms - Take to a Doctor for Completion ( Accepting after June 1, 2025)

• Step 3 - Read the PRMS ATHLETICS HANDBOOK 

• Step 4 - Complete the Boys Athletics Handbook Acknowledgement Google Form (Accepting after August 1, 2025)

IF THE ABOVE STEPS ARE NOT COMPLETED, STUDENTS WILL NOT BE ALLOWED IN ATHLETICS or  BE ABLE TO TRYOUT FOR ANY SPORT
