To submit the required online forms:
You will need your student's Last Name, First Name, and Student ID #
Do NOT put the "s" in front of their student ID #
Follow the directions to complete all of the electronic forms.
Forms that must be signed and returned to coaches:
If you have asthma and would like to keep an inhaler in your Girls Athletics locker at school, you must submit this form to the school nurse. Print and take with you when you go get your physical.

**The Pre-participation Medical History and Physical Form must be taken to a doctor to complete. You need to fill out the Medical History section before going to the doctor. Please do this after May 23, 2023 to ensure that the physical is good for the entire 2023-2024 school year.
Once you have completed the RankOne online forms, turned in your physical, and turned in the handbook acknowledgement page, then your child will be eligible to participate in athletics (this includes try-outs and practices during, before and after school).
