To submit the required RankOne online forms:
You will need your student's Last Name, First Name, and Student ID #
Do NOT put the "s" in front of their student ID #
Follow the directions to complete all of the electronic forms.
Please UPLOAD a copy of your Physical to the RankOne website
Forms that must be signed that are not in RankOne :
Girls Athletic Handbook for 2026-2027 - this is DIFFERENT than the Handbook in RankOne and it MUST be read and signed (electronic signature form is linked below)
Print out last page, sign, and return OR
Fill out this electronic form instead
If you have asthma and would like to keep an inhaler in your Girls Athletics locker at school, you must submit this form to the school nurse. Print and take with you when you go get your physical.
**The Pre-participation Medical History and Physical Form must be taken to a doctor to complete. You need to fill out the Medical History section before going to the doctor. Please do this after May 27, 2026 to ensure that the physical is good for the entire 2026-2027 school year.
Once you have completed the RankOne online forms, uploaded your physical to RankOne, and submitted in the handbook acknowledgement page, then your child will be eligible to participate in athletics (this includes try-outs and practices during, before and after school).
