Starting with the 2025 school year, we have a new process for reporting incidents and irregularities.Â
For incidents involving possible cheating:
Contact Assessment to discuss the situation and determine if the incident qualifies as cheating.
If it is determined that cheating occurred:
Note on the Score Code Form that the student's test will be 'O' coded due to cheating
You will be sent a form to fill out for TEA
For irregularities:
Contact Assessment to determine if the incident needs to be reported and, if yes, whether it is a Procedural or Serious Irregularity.
If a test needs to be 'O' coded, document on the Score Code Form with the reason.
Collect signed statements from anyone involved (for Serious Irregularities, these need to be on school letterhead). These will need to be uploaded to your Documentation Google folder. These statements should:
Document everything that occurred before the incident was handed off to the CTC.
Include the person's title, role during testing, steps they took, and amount of time that lapsed during the incident.
Explain the event clearly enough that someone with no prior knowledge could follow from start to finish.
You will be sent a fillable PDF to complete electronically based on type of irregularity:
Once the form is filled out, it will be sent to the campus Principal to sign. Both CTC and Principal will receive a copy of the completed form.
