Step 1:  Rank One On-line Forms (CLICK HERE)- You will need to access, sign and submit:

• Handbook Acknowledgement

• Medical History

• Parent Consent (English or Spanish)

• UIL Forms (English or Spanish)

 Step 2:  RRISD Sports Physical/Medical History/Athlete Contact Info- Note for physicals: All physicals must be completed on the   current up-to-date UIL Pre-Participation Medical History and Physical Form.  

Print the forms - fill out the Medical History form and take them to your doctor at the time of the physical exam. 

Step 3:  1) Read - Canyon Vista Athletics Handbook

              2) Complete - Athletic Handbook Acknowledgement Form
