Travel Reimbursement Form
To request reimbursement for travel from the LHTCN, please click the Travel Reimbursement Form link below. You will get a pop-up message informing you that it will make a copy of the form for you and that it will also copy the app scripts within the form. Click the "Make a Copy" button to confirm and then follow the directions in the form.
