**To advance to the district science fair you must have all forms completed and submitted when you turn in your science fair project. Please refer to the list below to make sure you have all required forms submitted.
Elementary and Junior EZ Form (NCSEF form replaces ISEF forms 1, 1A, and 1B): Identifies you and is required for you to participate. Each elementary and junior student, for both individual and team projects, must complete and sign the form. Each parent and Adult Sponsor must review the Research Plan and sign and date this Form.
Research Plan/Project Summary Instruction and Template-the instruction will tell you how to fill out your template. This should be completed before starting the actual experiment.
2025 NC Science and Engineering Fair NCSEF Participant Verification and Release Form: Attests you did the work, confirms the title of your project and category it should compete in, and indicates permission to share your name and photo on the NCSEF website. Each student and a parent/guardian must sign and date.
Checklist for Adult Sponsor: This form, required of all junior and senior projects, helps ensure students hard work can be recognized and prevents disappointment with a checklist for items and forms to review, especially those required based on the research topic.Adults sponsors should complete all sections, carefully reviewing the Additional forms required section checking only boxes applicable to the project, sign and date.
Approval Form: A separate form must be signed and submitted for each team member, signed by the student and their parent/guardian (not teacher), acknowledging that all rules have been read and followed.
Abstract: A 250 word or less summary written after experimentation is completed. Include a few sentences covering the purpose of the experiment, procedure, data collected, and conclusions.
ANY EXPERIMENTS INVOLVING PEOPLE, VERTEBRATE ANIMALS, HAZARDOUS CHEMICALS OR BACTERIA WILL REQUIRE ADDITIONAL FORMS. PLEASE SEE YOUR SCIENCE TEACHER IF YOUR EXPERIMENT INVOLVES ANY OF THOSE THINGS.
