Add RCC Event Form 

• Use this form to schedule RCC events. This replaces the Facility Request form but includes all of its information.

Add RCC Zoom Meeting 

• Use this form to schedule a meeting where everyone is on Zoom. 

Payment Request Form

• Use this form to request reimbursement or payment to a person or a business

• This form supports the prefill of information from the Add Form Prefill Info form.

Add Form Prefill Info 

• Use this form to add your name, address, email, phone and possibly other info that can be used to prefill the Payment Request Form using a unique code. (This might be enhanced to be used with other forms in the future.)

Test Prefill and Photos

• This is a test program specifically to verify the info that you have entered in the Add Form Prefill Info form.

• This form also introduces the new support for the Payment Request Form that allows you to take photos of receipts and bills. This makes it easier to fill out this form from a phone.
