As a newly hired employee of Rizal Med, please make sure that you submit the following at the HRMS Office for the immediate processing of your salary, enrollment in GSIS and updating of your Philhealth membership:
1. Landbank Account Number (processed at Pasig Capitol Branch only; please call their hotline numbers:(02) 633-9718, 632-7780, 638-0598 to verify their office hours)
submit the photocopy of your atm (front page only)
Note: Former Employees/Job Order Personnel, who have existing Payroll accounts processed at Pasig Capitol Branch, must also submit the front page photocopy of their ATM.
Kindly process your account via Landbank Online Account Opening before going to the said branch. Upon going there, bring 2 Government IDs.

2. GSIS and Philhealth (answers must be typewritten):
Download PMRF (print 2 copies in long bond paper, back to back)
Download GSIS (attach 1x1 pic, if with previous Government Service, indicate GSIS BP number and name of previous employer beside your signature)
3. Filled out Monthly Daily Time Record (DTR)
Note: For salary purposes, please manually fill out the this downloadable DTR on your first 2 months of employment, even when you are using the biometrics.
Without the manual DTR, no salary can be processed.
Submit the photocopy of your manual DTR to Ms. Jen of HRMS, on the 1st week of each succeeding month, as soon as it is signed by your supervisor
Submit the original copy of your manual DTR to your department/unit as reference for the Monthly Report of Attendance
NOTE: REQUIREMENTS 1 & 2 shall be submitted within 3 days upon your Assumption to Duty.
