The Red Lion School District recognizes there are occasions when it is important for school personnel to administer medication to students during school hours. When it is necessary for your child to receive medication during the school day, the primary care provider’s signature and specific directions must be provided to the school.
Any medication, prescribed or over-the-counter, must be brought to school by a responsible adult in the original container and must be labeled as follows:
1. Name of student 2. Name of medication 3. Dosage of medication 4. Time to be administered
(Students are NOT allowed to bring in the medication)
All medications will be kept in the health room. Your child should report to the health room when scheduled to take the medication.
For Inhaler/Epipen Self-Administration and permission to carry, (the order from the doctor needs to state, “Student is allowed to carry and self-administer medication in school”):
I authorize the Red Lion Area School District and its employees to allow my child to possess and use his/her asthma inhaler/epipen while in school, at a school-sponsored activity, before or after school hours.
I agree that my child will demonstrate to the school nurse the proper use and technique for self-administration of the asthma inhaler/epipen.
I agree that my child will notify the school nurse or qualified school personnel immediately following the use of the asthma inhaler/epipen.
I acknowledge that the school bears no responsibility for ensuring that the medication is taken or properly self-administered.
I agree that if my child abuses or ignores this privilege, school personnel may confiscate the asthma inhaler/epipen and they will lose the privilege to carry the medication.
NOTE: REQUESTS ARE EFFECTIVE FOR CURRENT SCHOOL YEAR ONLY AND MUST BE RENEWED ANNUALLY AND WHEN THERE IS A CHANGE IN PRESCRIPTION.
