ImPACT Baseline Testing
Impact Baseline is required every two years. If you took the baseline test in the 2023-24 school year, upload your completion confirmation to your clearance application.
Follow the ImPACT Baseline Testing Instructions to access your test.
Take a picture or screenshot of your ImPACT Completion Confirmation page.
Upload to your online clearance application.

PHYSICALS MUST BE DATED ON OR AFTER 5/15/24 in order to be valid for the 2024-25 school year.
Your profile must be updated each school year, but much of your information from the previous year will be retained
The Physical clearance form that your doctor will sign is included in your athleticclearance.com profile and can also be accessed at the bottom of this page.
