Sports Physical Form- Click Here- This must be completed before an athlete try's out for any sport at Central. It is good for 1 calendar year. Physicals can be done with a pediatrician or at a walk-in clinic listed. Please print the attached form.

Athlete Code of Ethics- Click Here- This document needs to be completed once athlete makes the team and before the first competition.

PrimeTime Application- Click on Flyer below- This document needs to be completed by all athletes. Students involved in Central sports will be a part of this after-school program to ensure their attendance and safety while at practice.
