If you would like a response or follow-up, include your name, phone number, and email address.
Select Your Role
Check the box that best represents your relationship to the district (e.g., Parent, Student, Community Member, etc.).
Type of Submission
Select the appropriate category:
Suggestion: An idea or recommendation for improvement.
Complaint: A concern requiring resolution.
Question/Concern: A general inquiry or issue not considered a formal complaint.
Compliment: A positive comment or praise.
School or Department Involved
Indicate which area of the district your submission relates to (e.g., Elementary School, Transportation, etc.).
Description
Provide a clear and detailed description. Include dates, names, and specific events if applicable.
Desired Outcome
Describe what resolution or response you are hoping for (optional).
Response Request
Indicate whether you would like a district representative to follow up with you.
If your submission is a complaint, it will be addressed following these levels:
Level 1 – Address It at the Source
Try to resolve the issue with the person directly involved (e.g., teacher, coach, staff member).
If unresolved, move to Level 2.
Level 2 – Principal
Submit this form to the building principal within 60 days of the incident.
Include a clear description of your concern and the outcome you’re seeking.
The principal will review and attempt to resolve the issue within 30 school business days.
Level 3 – Superintendent
If dissatisfied with the principal’s response, request a review from the Superintendent in writing within 15 days.
The Superintendent may meet with involved parties and will respond in writing within 10 days of the meeting.
Level 4 – Board of Trustees
If you believe district policy was not followed, you may appeal in writing to the Board within 15 days of the Superintendent’s decision.
The Board may choose to review the written record or hold a meeting to hear the matter.
The Board’s decision is final and will be issued within 30 days of their review.
