Immunizations

All Children enrolled in 4K and younger must meet Childcare Requirements, even if attendance is in a school setting.

Children enrolled in grade 5K or greater and enrolled in a childcare facility ( after school and/or summer program ) must meet school immunization requirements and have a valid SC Certificate of Immunization on file at the childcare and school facility.

Please provide a copy of your immunizations to Jackson Creek Elementary. You may upload to the enrollment page when you complete the Health Update Form, you may bring a copy to the school front office and/or you may email to me at aschemm@richland2.org
