Medication Permission Forms are found below.

• Morning medicines should be given at home.

• If an emergency medication or another prescription medicine is needed at school, the medication permission form must be signed by the provider and the parent before it can be given.

• All medications must be brought in by a parent. Prescriptions must have the pharmacy label intact and attached. Student information, medication name and dose, and directions must be legible.

• Students are not allowed to carry medicine unless additional permission forms are signed and only under certain circumstances. Please contact the health room if this is needed.

• Inhalers (with spacers if needed) should be provided for those students with asthma, not nebulizers.

• Avoid bringing in over-the-counter medicines unless absolutely necessary. If over-the-counter medicine is necessary, only a parent signature is needed on the medication permission form. We can not give more than the amount listed on the label.
