I'm excited to get to know you and your kiddo(s) coming to Dent Middle School! I know that you all have so much going on in your lives, so I want to thank you for making your child's health and safety a priority. Field Studies: If your child needs their prescription medication on a field trip, it will need to be in a labeled prescription bottle. The bottle should contain ONLY the dose needed for the field study.
**Please plan for the nurse to inspect and count the medication when you bring it to the health room.
Students are not allowed to carry medication of any kind with their personal belongings (including but not limited to: medication creams, nasal sprays, pills, inhalers)
If an over the counter medication (i.e. Tylenol) needs to be taken at school a medication permission form must be filled completely and signed by the parent and must be from the CURRENT school year.
Prescription medications must have a medication permission form filled out completely and signed by the parent AND the physician. Medications will not be administered without both signatures.
Due to Richland District Two policy, medications with aspirin require a physician’s signature.
Double check the form before you leave the doctor’s office :)
If your child needs to carry a life saving (ex: inhaler, epi-pen) or life preserving (ex: insulin, creon) medication with their personal belongings, physician permission must be given and additional paperwork must be completed. Please contact Nurse Butler should that be the case

Medication must be brought to the health room by the parent. Do not send the medicine with your child to school.
ALL medication must be in its original container. Prescription bottles must be labeled with the child’s name and date of birth.
If your child does not need the medication or needs to pause the medication for a time, the medication hold/discontinue form must be filled.
Please ensure your child's immunization record is on file with the school. If you are unsure, check with Nurse Butler. This is required by state law.
Students in the 7th grade or above are required to receive a Tdap booster. All records will be reviewed to ensure this is complete.
If your child is missing or has an incomplete record, you will be notified via mail and will have 30 days to correct or complete the record. If you fail to comply, your child will be required to stay home from school until this is complete.
If your child has a health concern (dental, vision, or otherwise) and finances are a burden, the school has numerous resources that may help your child. If you think you may qualify, please contact Nurse Butler.

If your child sustains an injury in the middle of the school year:

Crutches, scooters, wheelchairs or other assistive devices require a physicians order for use in the school.
"As needed" medications will require a medication permission form. (see above "Medication" section).
Accommodations needed will need to be discussed and approved by teachers and administration. Nurse Butler can help with this communication.
If your child has a fever of 100.4 they must stay home from school. They must remain at home until they are fever free for a full 24 hours WITHOUT FEVER REDUCING MEDICATION.
If your child tests positive for COVID-19 or Flu please notify Nurse Butler. A doctor's note will need to be submitted stating your child tested positive. If a home test is conducted for COVID-19, please fill out the Home Test Attestation Form (See to the left) and turn into the health room.
