Prescription Medication Forms

If your child takes prescription medication, they will need a trip-specific form filled out and signed by the physician. Please print out the appropriate form and take it to their doctor's appointment at any time during this school year. 

There are two options:
1. Student-Carry = The student carries and dispenses their own medication.

2. Chaperone-Carry = The student's chaperone carries and dispenses the medication.

Important Note: All controlled substances MUST be Chaperone-Carry!
