1. PRINCIPAL MEMBERS. A Principal Member is an employee who actively works full time and meets the requirement of the Client as an eligible member. The names of the Members must be in the list submitted by the Client to Intellicare.
Minimum and Maximum Enrollment Ages. A person who has attained the age of eighteen (18) years old but not more than sixty-five (65) years old. Attained age is defined as the age last birthday.
2. DEPENDENTS. Persons designated by a Principal for membership under this Agreement must be enrolled simultaneously or within thirty (30) days upon reaching six (6) months from the date of the Principal’s enrollment, except for cases involving marriage or the birth of a child wherein enrollment must be made within thirty (30) days from date of eligibility. Otherwise, a dependent can no longer be enrolled.
a) MARRIED PRINCIPAL. Hierarchy:
1st Priority-Legal Spouse, not more than sixty-five (65) years old;
2nd Priority- Legitimate biological, legally adopted or legitimized children at least thirty (30) days old and not more than twenty-one (21) years old who are unmarried, unemployed, wholly dependent financially upon the member.
b) SINGLE / UNMARRIED PRINCIPAL. Hierarchy:
1st Priority-Biological children who are unmarried, unemployed, at least thirty (30) days old and not more than twenty-one (21) years old who are unmarried, unemployed, wholly dependent financially upon the member;
2nd Priority-Parents not more than sixty-five (65) ; and
3rd Priority-Brothers, Sisters at least thirty (30) days old and not more than twenty-one (21) years old who are unmarried, unemployed, wholly dependent financially upon the member.
c) UNMARRIED PRINCIPAL WITH COMMON LAW PARTNER/SAME GENDER DOMESTIC PARTNER
1st Priority - Legitimate, legally adopted, or legitimized children who are unmarried, unemployed, wholly dependent financially upon the member at least thirty (30) days old and not more than twenty-one (21) years old
2nd Priority - Unmarried Common-law partner/ same-gender domestic partner (up to 65 years old)
Note: Parents and siblings of principal with domestic partner/common law/same-gender domestic partner will automatically be terminated upon enrollment of common law/same-gender domestic partner.
3. HIERARCHY RULE FOR SELECTING QUALIFIED DEPENDENTS.
Intelicare reserves the right to require at any time the submission of such documents which Intellicare may deem appropriate for the purpose of validating the eligibility of Principal Members and their Dependents.
Eligibility:
Meets the required tenure w/ client - tenure can be accumulated provided that none of the cancellation record, coaching log and/or memo is due to attendance, performance or behavioral issue
VAs who committed infractions will qualify once their case has been closed (6 months count will continue on the date of the case closure)
No pending cancellation or attrition risk
VA must be recommended by the Client Service Manager and/or department head
Process:
You will receive an email with details of HMO enrollment and a link to the enrollment form
Fill out the form and attach the required documents
Submit form
Member account number and HMO card delivery:
Enrollment usually takes 2-3 weeks before Intellicare sends us the member account numbers
Once received, HR will notify you with your account number
HMO cards usually take 3-4 weeks to process
Once received, HR will ship your HMO card to the registered address in the HR database
Eligibility:
Must meet the required tenure with FT client or 2 PT clients from the time you (principal member) were enrolled in our HMO program
Requests should be sent within 30 calendar days upon reaching the 6th month from member enrollment to HMO
Note: You will be responsible to monitor this. Late enrollments may be declined by Intellicare.
Process:
Fill out the HMO enrollment form (make sure to indicate for dependent enrollment)
HR will validate and confirm receipt of your enrollment request
Dependent account number and HMO card delivery:

Once received, HR will notify you with your account number

Once received, HR will ship your HMO card to the registered address in the HR database
