REQUIRED DOCUMENTS

The documents below may be required as part of your registration, depending on your unique situation. Not all documents listed below will apply to every child, the registration form will guide you regarding which documents to submit.

Liability waiver: Required for all participants, including siblings and friends
Most recent PLAY Project or DIR Floortime report: Required for children who received PLAY Project from a provider other than Relate & Grow within the last 4 months. If we do not receive a PLAY Project or DIR Floortime report for your child, we will need to set up a simplified DIR evaluation with you and your child to make sure we can provide the best service possible.
Latest ETR and current IEP: Required only if you want to apply for the Autism Scholarship. We will send you the enrollment paperwork and scholarship application or request for new provider once we receive this registration form.
Current IEP only: Required if the child has an IEP and we do not already have it on file.
Current IFSP only: Required if the child has an IFSP and we do not already have it on file.
If your child is not currently receiving PLAY Project or DIR Floortime from Relate & Grow, and you don't have a PLAY Project or DIR report created within the last 4 months, we will schedule a basic DIR evaluation online, at your home or another location and may request other progress documentation you may have available.
Other possible documents that could be helpful but are not required: Autism Scholarship application and proof of address if you already have a paper copy that you can complete and send; request for secondary provider if you already have a paper copy to complete and send; latest OT or ST therapy report; description of your child, their likes and dislikes; diagnostic report; medical records relevant to the program, etc.

REGISTRATION AND DOCUMENTATION DEADLINES

Documents must be RECEIVED by the following deadlines:

Enrollment documents deadline for children NEW to Relate & Grow: 3 weeks prior to start date of session. The enrollment packet will be sent to you once we receive this form.
Deadline for Autism Scholarship application documents: 3 weeks prior to start date of session.
Deadline for Autism Scholarship re-allocation and modifications: 1 week prior to start date of session.
Deadline for PAS authorization from Delaware County: 1 week prior to start date of session.
Deadline for submitting a recent PLAY Project report (for children who did not receive PLAY Project at Relate & Grow, but received it from another program): 2 weeks prior to start date of session.
Out of pocket payment deadline: 1 week prior to start of session.
Full registration and liability waiver deadline for all participants: 1 week prior to start of session.

Liability Waiver:

A liability waiver is required for every child participating in the group. Follow the instructions below to complete one for each child you are registering, then tell us how you submitted the waiver.

If you are unable to fill out the waiver at the link above, Click here to download and print a PDF. You must complete it by hand and upload the complete form to us. Once complete, click here to upload the file to our secure file inbox. Waivers for siblings or friends who are not going to receive therapy, can be sent via email to info@relateandgrow.com. Please do not email documents with private health information for a child that will be enrolled in the therapeutic aspect of the group. You may also mail us your waiver to the address listed in the description at the top of this section.

How to Submit Documents:
You can submit your documents in one of the following 2 ways:

1. Upload Documents Online: Please click here to upload any documents that have not already been sent in. You will need to enter a Google username and password to access our secure file inbox, if you do not have one and do not wish to create a login, please mail the documents.

PAYMENT DEADLINES AND PAYMENT METHODS

Your registration is not complete until we receive all required documents and payment. Please pay close attention to the deadlines listed below. Out of pocket payments are due 1 week before the start of the You&Me=We group session.
If your child is currently utilizing the scholarship or Delaware County funds with Relate & Grow for other services, we will let you know if any additional paperwork is needed. It is your responsibility to make sure that you will have enough funds available based on your yearly budget estimate and budget estimate updates*. If your are unsure of what you have available, contact us!
New participants paying with Autism Scholarship*: Confirmation that Relate & Grow has been added as a provider, signed re-allocation of funds form and modification forms are due 1 week before the start of the You&Me=We group session.
New participants paying with Delaware County Funding*: PAS and confirmation that we will be able to bill are due 1 week before the start of the You&Me=We group session

*Any amounts that we are unable to bill to the Autism Scholarship or Delaware County remain the responsibility of the family and will be billed directly to you.

We accept the following payment methods:

Payment via Zelle sent to monica@relateandgrow.com (free, no convenience fee). Please write "My Inner Sense" on the memo or note field.
$380 Payment online ($350 if registering in the month of April) by clicking here and selecting the correct amount (debit and credit card payments incur a convenience fee of 3%, it will be billed on a separate invoice. Bank payments are free).
Payment via check to Relate & Grow mailed to 5132 Schuylkill St. Columbus, OH 43220 (no convenience fee). Please write your child's name and "My Inner Sense" on the memo line.
Payment using Autism Scholarship or Delaware County Funds, if you selected either of these options earlier in the registration form, you should be aware of the deadlines and paperwork required. No payment is due at this time.
