Please see the chart to make sure you are elgibible for a work permit. If you have questions, email or make an appointment to see Mrs. Fairbanks. Students who drop below eligibility may have their work permit revoked!

Get hired! You must have a job to get a work permit.
Download and print the Work Permit Application (B1-1)
Complete the form, including your full social security # and signatures of Student, Parent and Employer (use pen, blue preferred)
After everything is completed, bring your application to the Career Center. **DO NOT EMAIL THE FORM**
Mrs. Fairbanks will review your eligibility and process the work permit request. Your signature is required on the work permit!
CAUTION: Treat your work permit application very carefully. It has personal information, including your social security number, on it. DO NOT email it (unless instructed to via encryption), leave it Mrs. Fairbanks' box, or on her desk, or hand it to other staff members. We take your personal information seriously--you need to as well!
