State of Illinois School Health Requirements for Freshman/New Transfer Students

The State of Illinois Certificate of Child's Health Exam form and proof of current immunizations are due *on or by the FIRST DAY of school in August* per Reavis High School District Policy. If you are unable to complete a physical exam and/or immunization schedule by the first day of school, you MUST provide a SIGNED notice from your child's physician with the scheduled appointment date (within 30 days of note) or immunization series schedule on or by the first day of school. Reavis High School will not accept electronic appointment reminders or unsigned appointment cards. Students with religious exemptions must provide the State of Illinois Religious Exemption form. In case of a disease outbreak, students with exemptions must be excluded from school for a determined time period as mandated by Illinois Department of Public Health (IDPH). 

If a child does not submit proof of having had either the health examination or required immunization, then the child will be excluded from school until such time as the child presents completed forms to the School Health Office. New Students to the district from out of state arriving AFTER OCTOBER 15th have 30 days from their first day of school to submit a health exam and proof of current vaccinations or immunization series schedule. 

9th Grade (Freshman)

Requirements

Parents/Guardians-Please make sure to complete and sign the medical history portion of the form on the second page. Without this section, the exam in considered invalid.

A dental exam must be performed by a licensed dentist and submitted prior to May 15th of the current school year

Must be performed by a licensed optometrist within one year prior to the first day of school and submitted before October 15th of the school year. Vision and hearing screening is mandated per IDPH and Illinois State Board of Education (ISBE) for all transfer students (Freshman are considered new to the district). 

*Please speak to the nurse or health secretary if a waiver is needed for eye or dental exams*

Sports Physicals: A yearly Sports Physical is required for all athletes to participate in school sponsored atheletic programs. These are turned into the Athletic Department. 

*9th graders do not need a separate sport physical. The required State of IL child health exam can be used as a sports phyiscal. Please note: a sports physical CAN NOT be used in place of a physical exam. Sports physicals are valid for 395 days from the date of the exam.
