Section 504

Disability. Under Section 504, an individual with a disability is defined as a person who: (1) has a physical or mental impairment that substantially limits a major life activity; (2) has a record of such an impairment; or (3) is regarded as having such an impairment.

The determination of whether a student has a physical or mental impairment that substantially limits a major life activity (and therefore has a disability) must be made on a case by case basis.

The 504 Process:

• Parent(s) make a request in writing for a 504 plan

• Parent submits medical evaluation documenting the nature of the disability

• 504 Coordinator requests Parent Consent to Review Records

• Parent(s) complete the parent interview form below

• Teacher completes teacher input form

• 504 Coordinator reviews academic records, observes the student, and completes a 504 assessment

• Parents and 504 Team meet to determine if there is a need for a 504 plan and, if so, what accommodations are needed
