Disability. Under Section 504, an individual with a disability is defined as a person who: (1) has a physical or mental impairment that substantially limits a major life activity; (2) has a record of such an impairment; or (3) is regarded as having such an impairment.
The determination of whether a student has a physical or mental impairment that substantially limits a major life activity (and therefore has a disability) must be made on a case by case basis.
The 504 Process:
Parent(s) make a request in writing for a 504 plan
Parent submits medical evaluation documenting the nature of the disability
504 Coordinator requests Parent Consent to Review Records
Parent(s) complete the parent interview form below
Teacher completes teacher input form
504 Coordinator reviews academic records, observes the student, and completes a 504 assessment
Parents and 504 Team meet to determine if there is a need for a 504 plan and, if so, what accommodations are needed
