NYSSB School Notification Form

School Notification Form - Schools will inform the NYS Education Department of their intent to establish or continue a Seal of Biliteracy program each year they intend to offer the Seal. When filling out the School Notification Form, please be aware that our District Data Coordinator is Karl Kania, Director of External School Operations. Mr. Kania collects all data for our district, including NYSSB data, and reports collected data to SIRS. He can be contacted via email: Karl.Kania@RCSDK12.ORG and his contact number is 585-262-8453.

The following updates have been made to this form:

• Deadline - The deadline to submit this form is December 1st of each school year.

• Subgroups– High schools will submit enrollment data on the following subgroups of seniors that may pursue the NYSSB:

• English Language Learners (ELLs)

• Former/Ever ELLs

• Never ELLs who speak a home language of English

• Never ELLs who speak a home language other than English

Submit this form no later than April 15 of school year

End of Year Data Form

All schools should make a final determination of which students earned the NYSSB by no later than July 15th. If there were changes in the students who were anticipated to earn the NYSSB between the submission of the End-of-Year Data form by May 5th and this final determination, the school must submit an amended End-of-Year Data form by July 15th.
