All student-athletes MUST have a current physical on file at the school in order to be eligible to participate in our athletic programs. This includes an Insurance and Consent Form and a History Form completed by the guardian[s] and athlete, as well as a Physical Examination Form that must be completed by a doctor. Physicals are good for a calendar year from the examination date on the form. Without ALL forms being completed and turned in, athletes will not be permitted to participate in our athletic programs.

Please do not have student-athletes' physicals faxed to Rome Middle School. We are having too many issues where these are being lost in transport. Athletes should hand a PAPER copy of their completed physical packet to their head coach. Physicals can also be brought and left in the front office with staff. Coaches and Office Staff will ensure that appropriate personnel receive it.
