Licensed Practical Nurse
Currently licensed as a Licensed Practical Nurse in New York State
One year of related work experience in a Skilled Nursing Facility/Assisted Living, preferred
Trained in basic first aid
Able to learn and follow pre-established Wellness Department procedures as a team member with the main focus on resident care and medication administration
Good verbal and written communication skills and personal initiative
Flexibility in being able to accept assignments regardless of location, conditions or hours
Physically capable of performing all assigned duties within a flexible work schedule
Ability to work in a multi-disciplinary team environment
Provide ongoing assessment of resident medical and mental wellbeing
Ensure residents needing assistance with self-administration is one who needs assistance to properly carry out one or more of the following: reading label, correctly interpreting label, correctly ingesting, injecting or applying medication and correctly following instructions as to the route, time, dosage and frequency
Ensure residents needing supervision and assistance with medications are provided with proper dosage at the designated time and observe and record that the resident takes the medication
