Group Entry Form
INSTRUCTIONS
*please read all the Rules & Guidelines before submitting any entries*
1. This web page is for people entering on the behalf of more that one person. If you are entering for one person or yourself, please refer to the Individual entries form.
2. Please download the group excel sheet and fill in ALL the required information.
3. Create a Zip file with all entries.
- Label each entry that has the author's name and publication blacked out "ENTRY TITLE_AUTHOR_Blackout"
- Label each entry that has the author's name and publications visible "ENTRY TITLE_AUTHOR"
- Save the Zip file so that you can upload it to the form below.
- If you are unsure about how to create a zip file please refer to the button bellow and follow the instructions.
4. Upload your completed excel sheet and Zip file to the form bellow and complete the payment.
