Equipment Acknowledgment

I acknowledge receipt of the equipment listed below and agree to the following responsibilities and terms:

For all equipment:

• I understand that while I may use the equipment for personal purposes, the primary purpose is for doing PWN-USA work. Given this, I agree to keep all equipment in good condition.
• Insurance for the equipment listed below will be provided by PWN. Please e-mail if there is damage to the equipment and we will help coordinate a replacement or repair.
• Lost or damaged equipment might not be replaced with the same model or may not be replaced at all.
• If I need help using the equipment, I will e-mail PWN’s Office Manager at anand@pwn-usa.org.
• If I resign/retire or am otherwise no longer an employee/contractor at PWN, I will return the equipment to the Oakland office, or, in the case of a chapter, to the chapter co-chairs, before the last day worked.

For Laptops, tablets, and phones:

• I agree to be mindful of what is downloaded on the cellphone and/or laptop. I will only download applications through the device’s app store and will be mindful only to download and open files from trusted sources.
• In order to maintain the integrity of PWN and comply with the copyright law, I will not download any unlawfully obtained materials (e.g., films that weren’t purchased).
• Protective cases. If this is a cell phone, I agree to use the provided protective cell phone case at all times. If this is a laptop, I agree to always transport the laptop in the provided protective case or one of my choosing (if applicable).
• Some projects you work on may be sensitive in nature, this is why we require the following:
  • Equipment must be password protected at all times
  • Recovery mode must be activated (if applicable). This will allow us to locate and/or wipe any data on the equipment in the event that equipment is lost or stolen. If needed, PWN staff can provide technical support.

By entering my name and checking the box on this page, I acknowledge that I have received a copy of this agreement. I have read or heard this contract read aloud, and understand the conditions of the agreement. My signature is my commitment to adhere to these responsibilities and terms.
