The forms for Epipens/ Inhalers are on the PVSD website and parents of students with asthma/ food allergies should already be familiar with them. Additionally, our nurses email parents each summer with the forms in case they are needed for the upcoming school year but they are attached below for your convenience.
Medication permission forms that have already been submitted to the nurse's office for the 23/24 school year can be used for the overnight Disney Trip. Please reach out to the high school nurses directly to obtain a copy for medication drop off.
Some students may have appointments coming up over the break during which they can get them filled out while they are there or may need to schedule an appointment over the break.
Most primary care providers that have an electronic portal do not charge to submit their own form on their own letterhead which we typically accept here and can attach it to the other form but the specific medicine administration times must be clearly stated.
