• If you'd like to take your student home from an away meet in your own car, rather than having them ride the bus back, we need this form

• WE RECOMMEND THAT ALL STUDENTS FILL THIS OUT JUST IN CASE -- YOU NEVER KNOW!

Student Self-Transport Form

• For students old enough to drive themselves OR students who might use a rideshare service (such as Uber, Lyft, or Waymo) to get to and from practice

• CONSIDER FILLING THIS OUT JUST IN CASE SO YOUR STUDENT IS COVERED FOR A RIDESHARE SERVICE!
