The district maintains information about employee’s emergency contacts in case of a health or safety emergency at work. This information is confidential and is only provided to others on a need-to-know basis. Employees are encouraged to add and update their emergency contact(s) regularly with Human Resources.
The Federal Child Abuse Prevention and Treatment Act (CAPTA) requires individuals in this profession to report known or suspected instances of child abuse and neglect. Please read and acknowledge the attached requirement.
The district covers the cost of Worker’s Compensation for its employees. Read and acknowledge the attached document listing steps to take if injured while on the job.
Throughout your course of employment here, the district will provide access to various technologies, software, and internet access to promote resource sharing, innovation and communication. Read and acknowledge the district’s Technology Code of Conduct - Acceptable Use Policy.
Employees new to the district shall receive information about health benefits directly from our Benefits Department to your new PUSD email. The email shall include plan options and instructions on how to enroll. Please note, enrollment must be made within 30-days of your start date. Read and acknowledge the attached Benefit form if you are being hired as a part time employee.
If you wish to have direct deposit, complete this form and send to the Payroll Department for processing. The payroll department is located in Room 112 at the District Office. You can also email them at certificatedpayroll@pusd.us or call them at 626-396-3600 extension 88360.
Important information about Direct Deposits:
> Only submissions via PUSD emails are accepted. Please submit Direct Deposit requests using your PUSD email address.
> When emailing the Payroll department your Direct Deposit form, remember to attach a copy of your driver's license or photo ID.
> All direct deposit requests go through county approval; therefore, direct deposits won't be effective until the following month's paycheck.
This form is to designate a recipient for any salary or monies owed to you by the district while employed at PUSD if you no longer exist to receive it.

All PUSD employees are required to submit proof of protection against COVID-19. Your COVID-19 vaccination card or weekly negative test results are required. Please ensure you submit a copy of your vaccination card to the Human Resources Office.
