Example: “The purpose of this study is to determine whether high school students completing an online nutrition module improve their eating habits, and if so, the degree to which those improvements are sustained over time.”

Recruitment plan (if needed for surveys or interviews)

Appropriate permission in writing
On letterhead and signed by an official
From an agency email address, not a personal Gmail account
If done through a site such as Facebook groups, you need to reference the site's Terms of Use

Once submitted, the request will first go to your thesis advisor, consultant/instructor, or supervisor for approval.
