Health Document Manager Account Directions --

Note: Your Health and Immunization Forms (to print, complete, and take to your provider) are located in your health account. 

It is required that you submit health verification (annual physical), all immunization requirements, CPR certification, Mandatory Reporter, OSHA, and HIPAA into the Health Document Manager Account for verification. The system is run by CastleBranch and is separate from your Background Check account. Any questions regarding creating and uploading the required items into your account should be directed directly to CastleBranch for assistance. Contact information is listed at the bottom of this page.

The required Health and Immunization forms are available in your acct (top of your account) for download.
