Have your provider fill out the Medication Authorization form if any OVER THE COUNTER or PRESCRIPTION MEDICATION will be needed at school.

Immunizations: Previous Non-Medical Exemptions need to be renewed every year after June 30th.  (see site for more information)

• INCOMING 6th GRADE: Tdap is a required immunization. Please check with your provider before the start of school to set up an appointment.
