You may only be on ONE team for the dodgeball tournament.
Use this form if you have a full team of 6 or a partial team. Only ONE person on your team needs to register. They will be labeled as the team Captain.
Captains are responsible for paying the $30 team fee.
Captains should help come up with a team dress theme (tutus, matching t-shirts, headbands, etc.)!

Cash/Check also accepted. Please turn cash/check into the Kinard Main Office. Checks made payable to "Kinard Wellness Team".
