Have your provider fill out the Medication Authorization form if any OVER THE COUNTER or PRESCRIPTION MEDICATION will be needed at school.

Have your provider fill out a Healthcare Action Plan if needed (see site for more information)

Immunizations:  Previous Non-Medical Exemptions, need to be renewed every year after June 30th.  (see site for more information)

• INCOMING KINDER: Please check with your provider before the start of school to set up an appointment for immunizations.
