ALL MIDDLE SCHOOL PLAYERS MUST UPLOAD A PHYSICAL AT THE TIME OF REGISTRATION TO PARTICIPATE.

NO EXCEPTIONS

The physical must have the three following specifications to qualify:

1.  A statement specifically clearing the student to participate in sports;

2.  A Doctor's signature; and

3.  The physical must be dated and unexpired.

Immunization records, medical visit summaries, or clearing a student to participate in afterschool activities will NOT be accepted as a physical. 

TODOS LOS JUGADORES DE LA ESCUELA INTERMEDIA DEBERÁN SUBIR EL DOCUMENTO DEL EXAMEN FÍSICO AL MOMENTO DE LA INSCRIPCIÓN EN LÍNEA PARA ASÍ PODER PARTICIPAR.

NO HAY EXCEPCIONES

El documento del examen físico deberá incluir los siguientes 3 requerimientos para ser aceptados:

1. Una frase del médico que especifique que el estudiante puede participar en deportes;

2. La firma de un médico; y

3. El físico debe estar fechado y vigente.

NO serán aceptados como un documento de examen físico, el registro de vacunas, un resumen de la visita médica, o una nota diciendo que su estudiante puede participar en actividades después de la escuela.
