Getting Started

To get started with a provider and begin treatment ... 

1.  Please contact the provider's office directly for scheduling and availability.  

2.  Your provider will send you a link to complete a Billing Information Intake Form, which can either be downloaded or completed and submitted electronically.  

3. When we receive your complete Billing Information Intake Form with a copy of the front and back of your insurance card(s), we contact your insurance company to check your eligibility and benefits.  

4. With that informaiton, we will send you a Good Faith Price Estimate by secure email within 3 business days.
