This is a screenshot of the New York State Unified Court System’s Family Court Forms page. I used this site regularly to access essential forms for my SIJS clients, including the 6-1, 6-1A, 6-3, and others. The forms are publicly available at:

I followed this bill during the summer, as it impacted the work I was doing. Clients had questions about it, and I wrote a summary for my supervisor to help clarify its effects as apart of a project.
