For referrals:

If you have someone that you feel might benefit from a visit from our team, and they are open to us visiting them, please contact us at 503-501-1231.

If we don't answer, please leave us a VM with:

• your name and a return contact
• Some identifiers of the individual you are calling about (name, description, where we might locate them, color of text, cross streets, etc)
• What is the concern you are calling about?
• Is the person is open to our team reaching out to them (if you spoke with them)?

We will do our best to look for this individual when we are next out on rounds or can mobilize a team to reach out to them.
