Use this form if you wish to pick your athlete up from an athletic event. 

Fall Sports Information Night was Wednesday, September 3rd. Thanks to all the families who attended. Here is the general presentation for those unable to attend. If you have any questions about a specific sport, please contact or have the student speak with that coach (listed in the presentation). Good luck to all our fall student-athletes! Go Highlanders! 

ATHLETIC MEDICAL REQUIREMENTS: Before each athletic season (Fall, Winter, Spring), athletes must fulfill two requirements described below:

1) PHYSICALS: All student-athletes must have documentation of a current physical on file with the school nurse. Physicals are good for 13 months from the date of the exam. Students with expired physicals will not be permitted to participate with their team until a new exam is complete and on file with the school nurse. Please contact the school nurse to assure that your child’s physical is up to date and that current records are on file with the school nurse.

2) FinalForms: We are very excited to announce that the Portland Secondary School Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time. 

You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

Click on the Parent Playbook (attached) to get started in FinalForms. If you require any support during the process, scroll to the page bottom and click “Get Help”.

We are asking the parents/guardians of all students planning to play sports to create accounts in FinalForms and register your student(s).
